| Welcome to the Craftsbury Community Care Center, a Level III residential care home located in the heart of the Northeast Kingdom. Our residents enjoy spacious private units with full bath, emergency-call system, and a patio or a porch. Nutritious meals are served in our sunny commons, three times a day. The Center also provides housekeeping and laundry services, assistance with personal care and medications (if requested), nursing overview and 24-hour supervision. |

| The mission of St. Johnsbury Health and Rehabilitation Center is to provide the finest quality health care, rehabilitative and support services to our residents and their families. Our care approach is patient-centered and involves an integrated program of therapies that address the total well- being of our patients. For each resident, we create an individual care plan and assemble a care team that may include specialists from several areas. The goal is to design a course of treatment and therapy that best meets the needs of the resident. |
