Concord Health Center, a division of Northern Counties Health Care, provides comprehensive health care services to our friends and neighbors in the Northeast Kingdom and we welcome the opportunity to be YOUR Medical Home!

Northern Counties Health Care offers patient centered health and dental care services, discounted prescriptions through the Community Health Pharmacy, home health and hospice services, and a sliding fee scale for those that qualify.

With over 40 years of providing health and dental services to our neighboring communities, we are happy to welcome patients at any of our locations. 

Northern Counties Health Care has 5 primary care centers and 3 dental centers:

Primary care: Concord Health Center, Danville Health Center, Hardwick Area Health Center, Island Pond Health Center, St. Johnsbury Community Health Center.

Our Dental:  Island Pond Dental Center, Northern Counties Dental Center, and Orleans Dental Center.

Our Caledonia Home Health Care & Hospice division offers clinical home care, personal care assistants, physical therapy, social services and hospice care and more.

Wednesday: 7:30 a.m. - 7:00 p.m.
Thursday: 7:30 a.m. - 7:00 p.m.
Friday: 8:00 a.m.-5:00 p.m.
